| Directions for completing the Roster/Payment Form

Below are the forms in Excel and PDF and a sample completed form for your review.
If you complete the form in Excel you can enter the cost for each cell and it will total each column and the total payment due. If you wish to add a column for payment per person you are welcomed to do that too. (It might make it easier for totaling what each person owes.)
If you use the pdf you will have to manually complete the form and total each column.
Columns to complete:
Tournament Dues must equal $850- $880
We are asking each club to purchase to DVD package for $30 to assist in offsetting the cost of the high end video production equipment that we are renting for the tournament. The package will include every game that is played in pool 1. This equipment will result in production quality video that can be used in television productions. As in 2005 we will have the games in pool 1 on the Jumbrotron.
Banquet Dues- $28 per adult
$10 per child (10 & under, or a young looking 11, 12)
Beer Dues- $10 (We’ll buy as many kegs as we can with the money we receive)
T-shirt - $15 (We are pre-paying for t-shirts. See website for Graphics)

USOA Fees/Waiver- Every entrant should have their dues and signed waivers sent to Carol Rose prior to the tournament (postmark date of June 11, 2007). Anyone signing the waiver and paying dues at the tournament will be assessed a $10 charge per person in addition to their USOA dues.

The Minnesota Waiver and both Video Waivers can be submitted on Friday, July 6, prior to the start of the tournament. Please have one representative from your club bring all the waivers to the “Loon Bin” (The information table set up near the diving well.).
You can send one check or multiple checks. (We prefer one check). Just verify the checks are made out to “Minnesota Underwater Hockey” and they add up to the total due.
Payment is due June 11, 2007. There will be a $100 fee assessed to late entries.
